OverviewServes as a highly visible, energetic leader and champion for patient and resident safety throughout all divisions of the organization. Responsible for oversight of the safety event reporting system, assures optimization of the system, compliance with data submission requirements for the Patient Safety Organization, and oversight of the modules that include safety event reporting, patient feedback, RCA, Peer Review, Mortality Review, and Covid Central exposure. Additionally is responsible for the ongoing design, development, and implementation of the organization's safety program, education and training materials. Promotes systems thinking and high reliability for recommended evidence-based safety behaviors and practices assuring a focus on people, process, and technology to improve performance. The System Director for Patient Safety is a content expert on patient safety, resident and environmental safety, change management, human factors engineering, systems and risk-based thinking, quality and safety improvement methods, tools and strategies. Plans, develops, and implements improvement projects focused on short and long-term improvement, and is able to scale improvement to impact the company. Coaches and mentors leaders, clinical and non-clinical team members throughout the organization providing direction, guidance, and leadership for effective change management, teamwork, and achievement of established goals and objectives. Works closely and collaboratively with medical, nursing, and clinical leadership to implement improvement strategies to continually improve safety.
What you will do- Applies system thinking and accounts for human factors in all aspects of safety programs. Designs and implements safety programs that raise awareness and promotes just and safe culture principles.
- Promotes safety culture. Designs and implements system wide efforts to assure just culture and safety culture principles and practices are supported and evident throughout. Coordinates system wide annual assessment of safety culture and assures appropriate distribution and communication of safety culture results. Collaborates where appropriate to develop, implement, and monitor action plans based on safety culture results.
- Works collaboratively with rCare Program Manager to assure all components of the safety event reporting systems are proactively managed, and provides oversight and optimization of the reporting system, work processes, and business rules.
- Assures robust processes for safety risk detection, capture, classification, analysis, prioritization, investigation, formulating, and implementing solutions and improvements, and monitoring for sustainment. Implements and monitors patient safety solutions and corrective actions. Assures just culture principles, robust and standardized methods, and tools for event investigation such as RCA and ACA, and work product.
- Oversees safety data management, assuring data is accurate, timely, and displayed in a meaningful way to end users. Additionally oversees safety data analysis, assuring appropriate data visualization tools and methods are utilized, such as basic statistical techniques to present data (e.g., mean,, standard deviation) and use of statistical process control chart (e.g., common, and special cause variation, random variation, trend analysis).
- Serves as the content expert on navigation and use on safety data applications. Interprets data to support decision making (e.g., benchmarking, outcome data). Assures action is taken when improvement opportunities are identified.
- Identifies improvement opportunities based on analysis of facility and entity data, emerging national, state, and local quality of care trends and issues. Selects appropriate safety, quality, performance and process improvement methods and tools. Facilitates the use of process analysis tools to display data (e.g., fishbone, pareto chart, run chart, scatter gram, control chart). Provides oversight of safety and quality related improvement projects and efforts, assuring alignment and coordination throughout the facility. Facilitates the establishment of priorities for safety and quality improvement activities and development of safety performance improvement action plans and projects.
- Assures safety program development, evaluation, planning, projects, and activities. Assures development or selection of process and outcomes measures. Assures evaluation/selection of evidenced-based practice guidelines. Facilitates development of safety related policies, practices, workflow, and guidelines incorporating safety behaviors, and human factor design and engineering. Evaluates readiness for participating in safety programs, data bases, certifications, and awards.
- Identifies content and design for just culture and safety related training programs. Assures appropriate and current content for facility education, training, and communication related to safety management. Assures alignment and coordination of safety programs with facility wide quality and safety program.
- Provides training on just culture, safety, and performance/process improvement, program development, and evaluation concepts. Evaluates effectiveness of performance/process improvement training.
QualificationsEducation- Masters Degree, in healthcare, behavioral science, or related field (Required)
Experience- 5-6 years relevant experience in hospital care quality, accreditation, safety, and/or risk management. ()
Skills and Abilities- Knowledge of clinical quality, patient safety performance measures and indicators, data definitions and sources, and relevant national databases and benchmarks.
- Knowledge of healthcare quality, safety, and satisfaction data design, collection, aggregation, and summarization.
- Expert knowledge of performance improvement models, methods, and systems required.
- Experience in project development, project management, change management, data management, and data analysis.
- Excellent oral and written communication and presentation skills. Ability to express and document situations and processes.
- Demonstrated leadership on projects providing subject matter expertise and technical direction.
- Ability to collaborate with others to troubleshoot and resolve issues. Organized and adaptive.
Licenses and Certifications- Certified Professional in Patient Safety (CPPS) - Institute for Healthcare Improvement (IHI) within 90 Days(Required) or
- Certified Professional in Health Care Risk Management (CPHRM) - American Hospital Association Certification Center within 90 Days(Required)
- Registered Nurse (RN) - State Department of Health Professions (Preferred)
- Certified Professional in Healthcare Quality (CPHQ) - National Association of Healthcare Quality Healthcare Quality Certification Board (Preferred)
- Healthcare Accreditation Certified Professionals (HACP) - Center for Improvement in Healthcare Quality (Preferred)
- Lean Six Sigma Yellow Belt - Six Sigma Global Institute (SSGI) (Preferred)
