About the role
The Recovery Navigator will play a critical role in delivering the IMPROVE-65 intervention - a patient-centred, evidence-based, multifaceted recovery program designed to optimise care and outcomes for older adult’s post-hospitalisation. This position involves coordinating care between acute hospitals and general practitioners (GPs), conducting telehealth consultations, and supporting patients through goal setting and recovery planning.
Key responsibilities include:
Assessment of patients’ barriers and challenges to recovery
Conduct patient reported outcome assessments
Assist participants to identify achievable recovery goals
Liaise with GPs
Conduct Scheduled telehealth check-ins to monitor patient progress, recovery milestones and emerging needs.
Adhere to trial protocols
About you
The successful applicant will bring knowledge and relevant experience and to excel in this role supporting and promoting Cabrini Research.
You will have:
Registered Nurse, Allied Health Professional; or equivalent clinical background
Experience in patient assessment and care coordination
Strong communication skills for liaising with patients and GPs
Proficiency in telehealth technologies
Knowledge of geriatric care and rehabilitation
Familiarity with functional assessment tools
Ability to create patient-friendly educational materials
