Where You Will Work: 

You will work within the Neurosciences service line, with primary office location on the unit, supporting system-wide stroke program operations. This role focuses on guiding stroke patients through their care journey-from stroke activation through post-discharge- to ensure continuity of care, patient understanding, and optimal outcomes.  Furthermore, this role focuses on providing education, and ensuring patients receive the resources needed to achieve the best outcomes. 

Expectations We’re Looking For: 

We are seeking a motivated, detail-oriented experienced registered nurse to support and advance our system-wide Stroke Program. This role requires a strategic thinker who can function autonomously while influencing outcomes across disciplines. The Stroke Navigator will:  

  • Monitor, analyze, and report key stroke quality metrics (e.g., door-to-needle times, thrombolytic compliance, dysphagia screening, core measures) 
  • Lead multidisciplinary performance improvement initiatives to optimize patient outcomes 
  • Ensure ongoing regulatory readiness and compliance with stroke certification standards 
  • Coordinate stroke committee meetings, case reviews, and action plans 
  • Collaborate with Emergency Department, ICU, Radiology, Neurology, Rehabilitation, and system partners to ensure continuity of care 
  • Guide stroke patients through their hospitalization and transition of care.
  • Serve as consistent point of contact for patients and families.
  • Provide clear, ongoing education on stroke diagnosis, treatment, and recovery with direction from provider documentation
  • Reinforce stroke warning signs and prevention strategies
  • Ensure follow-up appointments are scheduled prior to discharge when possible
  • Coordinate with outpatient providers and services
  • Conduct follow-up phone fall to assess patient status, address concerns, and reinforce education
  • Help remove barriers to care
  • Focus on interventions that improve stroke outcomes and reduce readmissions
  • Support and help promote the Stroke Support Group
  • Assist in connecting patients and families with community resources post-discharge
  • Registered Nurse license issued by the state in which the team member practices. 
  • Bachelor's Degree in Nursing Required
