Schedule Details/Additional Information:

Employee will work at least 40 hours within a standard two-week pay period upon successful completion of initial onboarding.

Pay Range

$26.55 - $39.85

Essential Functions

  • Coordinate and conduct patient assessments utilizing community paramedics for: Community, 9‑1‑1, and third‑party referrals
  • Qualifying inpatient discharges prior to hospital release
  • Collaborate with hospitals and facility staff to support on‑site discharge planning and care transitions as needed
  • Work closely with physicians, PA‑C’s, RNs, behavioral health providers, case management, pharmacy, and insurance plans to ensure continuity of care
  • Perform SDOH and inpatient assessments to identify clinical, behavioral, social, and environmental needs
  • Develop, implement, and monitor individualized care and aftercare plans
  • Facilitate referrals to primary care, behavioral health, community‑based case management, social services, faith‑based, governmental, and third‑party agencies
  • Coordinate resource management for patients and families across the continuum of care
  • Provide patient and family education related to clinical conditions and care plans
  • Implement and support appropriate clinical interventions delivered by community paramedics to achieve optimal outcomes
  • Manage complex, high‑risk, high‑utilizer, and non‑compliant patient cases, including crisis response
  • Develop targeted outreach strategies for patients lacking consistent primary or behavioral healthcare engagement
  • Identify barriers to care and collaborate with patients, families, and support networks to establish integrated plans of care
  • Participate in referral conferences, interagency meetings, treatment planning, and departmental initiatives
  • Ensure timely, accurate case management and documentation in accordance with established standards
  • Identify system and provider issues and contribute recommendations for service and process improvement

Education, Experience, & Certifications:

Required: Bachelor’s degree in nursing or related field required.

Preferred: Master’s degree in nursing, social work, or related field. Current NCOEMS certification as EMT, AEMT, or Paramedic. Prior experience in behavioral health or SUD, experience with community-based case management, and behavioral health. experience in behavioral, clinical, utilization management, home care, discharge planning, and/or case management required.
