Major Responsibilities: The patient navigator guides patients throughout the cancer care continuum, from cancer prevention, to treatment, to survivorship, to end-of-life care. 5)Facilitates linkages to follow-up services. 1)Coordinates appointments with providers to ensure timely delivery of diagnostic and treatment services. 2)Ensures that appropriate medical records are available at scheduled appointments. 3)Arranges transportation assistance with available resources. 4)Coordinates appointment with financial navigation services. Patient Engagement and Experience 1)Maintains communication with patients, survivors, families, and health care providers at various touchpoints during their cancer care. 2)Monitor patient satisfaction with the cancer experience and works with team(s) to address any identified issues or barriers. Communication and Physician Partnership. 1)Communicates and collaborates with the treating physician(s). 2)Communicates and collaborates with the multidisciplinary team(s). 3)Receives and responds to new patient referrals. Professional Role and Associate Engagement. 1)Provides non-clinical and emotional support for patients. 2)Advocates on behalf of the patient and Empowers the patient to self-advocate. 3)Maintains own knowledge, ability, skills, competencies. Education/Experience Required: •Bachelors’s degree in health care related field preferred •2+ years of experience working in a medical setting with oncology patients and families. Knowledge, Skills & Abilities Required: •Global perspective of changing health care environment •Ability to prioritize work •Proficient in computer skills •Effective interpersonal communication skills •Knowledge about the breadth and accessibility of community resources. •Ability to work independently. n/a Physical Requirements and Working Conditions: • Flexible hours according to customer needs • Ability to demonstrate a commitment to patient-focused care • Ability to prioritize independently and respond to multiple simultaneous requests • Ability to work in a fast-paced environment with established time constraints and in difficult situations • May be exposed to hazardous materials and life-threatening diseases • Ability to perform lifting/transfer activities related to patients (up to 50 lbs. may be required) • Must be able to travel to visit physicians and other practices within our region • This position needs to recognize needs and behaviors of specific age groups of patients treated If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
