Job Description

ESSENTIAL FUNCTIONS AND JOB RESPONSIBILITIES:   

  • Provide continual navigation support for breast care and cancer patients and their families. 

  • Assist patients with transition from diagnostics through treatment by coordinating appointments through advocacy and education. 

  • Educates patients about their disease, prescribed treatment, and side effects of treatment and how to manage them 

  • Provides clinical resources- where to seek a second opinion, treatment, and how to access reliable sources of information 

  • Addresses identified needs and barriers of care through available programs and support systems.  Identifies areas of individual strengths for patients to use empowering them to help themselves.   

  • Answers questions as appropriate and serves as liaison to the physician for additional questions or concerns 

  • Functions as a member of the interdisciplinary teams, Breast Program Leadership Committee and the MBCC. 

  • In consultation with other members of the healthcare team, promote awareness of clinical trials as appropriate. 

  • Supports timely scheduling of appointments, diagnostic testing, and procedures. 

  • Educates and identifies patients for genetic counseling or fertility preservation and facilitate appropriate referrals. 

  • Maintains Breast Cancer Registry in accordance with national standards and presents the data to the BPLC. 

  • Coordinates Lymphedema treatment, referrals to Community Support Services. 

  • Provide education to patients enrolled in clinical trials. 

  • Follows the survivorship protocols including the formation of survivorship letters and will contribute to our goal of improving health, wellness and the quality of our patient’s life. Assists patients with breast disease or breast cancer, in compliance with the continuation of therapies per NBCC standards.  

GENERAL QUALIFICATION STANDARDS & JOB REQUIREMENTS: 

Education/Certifications/Licenses:   

  • Patient Navigators are navigation professionals who do not hold current healthcare licensures. 

  • Competency-based training and education in patient navigation for patients with breast disease or breast cancer from a qualifying training program.   

  • Knowledgeable about the breadth and accessibility of community resources  

Pay Range: $23.56 - $28.99 Hourly

The provided compensation range is based on industry standards and salary determinations will be made based on numerous factors including but not limited to years of experience and location of position.
