Why this role matters

The Outreach Case Worker play a vital role in supporting individuals who are experiencing or at risk of homelessness. By providing person-centred, trauma-informed and strengths-based support, this role helps people address the underlying issues contributing to homelessness and move toward stability, safety and long-term housing. Your work will contribute directly to breaking the cycle of homelessness and improving the wellbeing of some of the most vulnerable members of the community.

About the role

Based at The Third Place in Subiaco, the Outreach Case Worker provides case management support to individuals connected to the Subiaco Engagement and Assessment Team (SEAT) hub.

Working one day per week (Wednesday), you will engage with individuals experiencing homelessness, undertake assessments, develop case plans and provide coordinated support that promotes self-determination, safety and long-term stability.

This is a 6-month fixed term position, with the possibility of extension subject to funding and service needs. Pay is in accordance with a SCHADS Level 5.

How you will make an impact

  • Provide case management support using a strengths-based, trauma-informed and person-centred approach

  • Engage with individuals experiencing homelessness, including those sleeping rough, to build trust and understand their needs

  • Undertake risk assessments and needs identification using approved tools and documentation

  • Provide information, referrals and service linkages to support individuals in addressing underlying issues contributing to homelessness

  • Support individuals to access housing, income support, mental health services, physical health care and other essential supports

  • Maintain accurate and up-to-date records, data and case notes in line with TSA standards

  • Collaborate with internal teams, external agencies, government services and community partners to ensure coordinated care

  • Contribute to a safe, inclusive and supportive team environment

What you will bring

  • Diploma in Community Services or a related discipline, or significant experience in homelessness or welfare service delivery

  • Practical experience supporting individuals experiencing homelessness, disadvantage or complex needs

  • Ability to assess risk, respond calmly in challenging situations and manage complex behaviours

  • Strong communication skills and the ability to build rapport with individuals who may be non-engaging or experiencing crisis

  • Understanding of trauma, mental health, substance use and the systemic challenges faced by people experiencing homelessness

  • Commitment to ethical practice, professional boundaries and collaborative teamwork

  • Current Senior First Aid Certificate

  • Current Driver’s Licence
