Job Description Summary:

Monday - Friday, No Weekends and No Holidays

OhioHealth Palliative Care Clinic is seeking an experienced Outpatient Registered Nurse to join our innovative, team‑based care model. In this role, you will support patients with advanced cancer by providing compassionate, longitudinal care focused on symptom management, education, and quality of life.

As an integral member of an interdisciplinary team, including physicians, advanced practice providers, social work, pharmacy, medical assistants, and office support, you will serve as a continuity anchor for patients and families. Responsibilities include triaging patient calls and messages, coordinating symptom management follow‑ups, providing patient and caregiver education, and supporting goals‑of‑care discussions in collaboration with providers. This role offers the opportunity to build meaningful relationships with patients over time while practicing at the top of your license.

The ideal candidate brings prior experience with serious illness, preferably in oncology, palliative care, or hospice, and is comfortable working in a growing, dynamic clinic environment. You will receive a structured orientation of approximately four weeks, including clinic onboarding and oncology clinic shadowing. This is a unique opportunity to help launch a new palliative care program at OhioHealth and be part of a passionate, patient‑centered team committed to delivering high‑quality, whole‑person care.


The Palliative Care Clinical RN will be a resource to patients/families, hospital staff and the palliative care interdisciplinary team.  This person will educate patients and families on palliative care, provide comprehensive assessment to patients, and clearly communicate needs to Palliative Care providers as well as floor staff.

Responsibilities And Duties:

-             Provides clinical care within the RN scope of practice, documents assessments and reports moderate to distressing symptoms to Palliative Physicians/APPs and floor nurse
Follow up on hospice GIP patients as needed
-             Able to participate in advance care planning discussions with patients and families and completes documentation (advance directives)
-             Communicates plans of care to referring providers and unit clinical teams and ensures recommendations are implemented
-             Provides education to patients and their families about palliative care
-             Functions as a resource and support to patient care units, and patient care staff regarding palliative care including medication delivery and end of life care
-             Identifies learning needs of patient care staff and communicates these to Palliative leadership
-             Provides education to hospital staff on Palliative Care as requested
-             Professional communication skills with providers, hospital staff, patients/families, internal and community resources

Bachelor's Degree (Required)

Additional Job Description:

SPECIALIZED KNOWLEDGE

-             3-4 years’ experience in working in academic or large community hospital in a related clinical area (oncology, geriatrics, ICU, neuro), knowledge of palliative care

-             3-4 years palliative care/hospice experience

-             CHPN specialty certification
