This fully remote position supports all service lines within the Continuing Health Enterprise including, but not limited to, Home Health, Hospice, and Rehab. The role focuses on improving quality and clinical outcomes through a hybrid of performance analytics, education, and quality improvement initiatives. The position develops meaningful measures and reporting, translates data into actionable insights, and supports interventions and ongoing monitoring aligned to quality, safety, regulation, and value-based care.

Schedule: Monday through Friday normal business hours

Pay Range

$35.50 - $53.25

Major Responsibilities: Clinical Documentation Accuracy 1)Facilitates the monthly branch OASIS Health Outcomes Committee meeting to discuss branch specific data and goals 2)Collaborates with other home care departments to eliminate duplication and ensure compliance with timeliness standards supportive of achieving submission deadlines 3)Attends branch-specific meetings monthly to develop relationships of trust with each branch management and staff supportive of achieving the Health Outcome goals 4)Responsible for accuracy and timeliness of clinical documentation. 5)Reviews all Oasis assessments for inconsistencies, omissions, and errors. 6)Works with Clinical Managers to ensure that all patient care documentation is timely and meets regulations. 7)Reviews SHP Oasis alerts for resolution Data Analysis 1)Prepares clinician specific statistical report summaries to identify educational needs to include Oasis errors, adverse events, and case weights 2)Works with clinical managers to improve field associate performance on Oasis indicators 3)Participates in quality/performance improvement activities as directed 4)Reports documentation timeliness and documentation for appropriate developmental activities Reimbursement Validation 1)Monitors documentation closely for quality, timeliness and accuracy to assure all Medicare and third- party payor reimbursement guidelines are met. 2)Monitors Oasis answers to assure accuracy, which impacts Case Weight-Medicare revenue per case 3)Collaborates with other home care departments to eliminate duplication and ensure compliance with standards related to timely assessment submission Education/Training 1)Coordinates and implements standardized, consistent Oasis educational modules to be delivered to associates at all offices. 2)Provides education to clinical field associates at time of hire, as well as ongoing during associate tenure to improve Oasis knowledge 3)Participates in just in time training for associates as appropriate Licensure: Nurse, Registered (RN) OR Physical Therapist Education/Experience Required: • Registered Nurse, Physical Therapist and/or HIT • 3 years appropriate clinical experience • 2 years of homecare experience • Intention to complete OASIS -D education and submit for D certification Knowledge, Skills & Abilities Required: Exceptional interpersonal skills--able to work well with diverse personality types in a fast paced environment --internal & external customers. •Strong knowledge of home care services • • Illinois License or accreditation Physical Requirements and Working Conditions: 1. Ability to travel between AHHS branches in types of weather on all types of road conditions. 2. Good analytical skills 3. Excellent interpersonal/communication skills 4. Must be knowledgeable of regulatory home care guidelines and OASIS content If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
