SUMMARY

Implements and documents treatment programs. Provide superior customer service by modeling the Brand Promise and Core Values.

Unity Living Center focuses on providing treatment and rehabilitative care for patients with medically complex needs, dementia, and behavioral challenges. Special services include short-term rehabilitation, pain control, IV therapy, peritoneal dialysis, access to hemodialysis, ventilator beds, and relaxation therapy to meet the needs of those with chronic diseases.

SCHEDULE: Day Shift/Varies

ATTRIBUTES

  • Associate's Degree or Certificate in Occupational Therapy or Physical Therapy from an accredited school. 

  • Rehab Only: Successful completion of all fieldwork requirements and eligible to sit for the PTA or OTA exam. 

  • HHCS Only: Minimum of 2 years post-graduate experience as a COTA in an acute care setting and/or an acute rehabilitation facility.

  • Reads, writes, and speaks English. 

  • Excellent interpersonal skills. 

  • Computer skills required. 

  • Required Licensure / Certification Skills:

  • Licensed and/or registered by the State of New York.

RESPONSIBILITIES

  • Within standards of practice established for COTAs and PTAs, completes and documents assessments, establishing objective, achievable, and measurable long and short-term goals.

  • Implements treatment programs individualized to the patient's needs and discharge plan; issues and provides instruction in the use of adaptive equipment; documents patient response to treatment according to department policy. Communicates directly with the therapist regarding the patient’s plan of care, concerns, or any observed changes in medical status.

  • Participates in discharge planning, performs timely and adequate family and staff teaching to facilitate the discharge plan. Offers appropriate suggestions for equipment needs post—discharge. Communicates home programs in a neat, concise, and organized manner.

  • Coordinates patient schedule with team members, readily and effectively adjusts schedule to meet changes in census, staffing, patient needs, and space constraints. Documents patient charges and attendance at the end of each day.

  • Maintains professional growth through active participation in continuing education programs, departmental in-services, professional organizations, and review of professional literature.

  • Reports problems and concerns and consults with the Supervisor regarding clinical issues and decisions.

  • Fosters team, patient, and family communication. Communicates pertinent patient information to all members of the health care team.

  • Communicates pertinent information to patient and family members, including goals, procedures, progress, and prognosis, as appropriate.

  • Displays the capacity for motivating others. Recognizes the effects of his/her own non-verbal behaviors upon others. Correctly interprets and responds to the non-verbal behavior of others.

  • Rehab Only: Practices safe and economical utilization and distribution of department equipment, supplies, space, and utilities. Maintain accountability for equipment issued.

  • HHCS Only: Maintains required agency visit productivity.

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
