Role title: Occupational Therapist — Transitional Aged Care Programme
Role type: Two positions available — Maximum-term Part-time (32 hours per week) until June 2027, and Maximum-term Full-time until February 2026
Role grade: Grade 1-2

Salary: $73,427.07 - $104,752.76 (Pro-rata) + Super

About the role
- Provide comprehensive Occupational Therapy assessment and evidence-based treatment for clients on the Transitional Aged Care Program in the community.
- Identify patients’ occupational needs through standardised and functional assessments, designing tailored, patient-centred interventions focused on therapy, discharge planning, and efficient patient flow.
- Manage cases effectively, liaising with community services, healthcare providers, and support networks to ensure coordinated care and appropriate referrals.
- Actively contribute to quality improvement and research initiatives within the Transitional Aged Care Program.
- Educate the Transitional Aged Care Team and other community groups, while responsibly managing occupational therapy equipment and organisational resources.
About you
- You’re a qualified and AHPRA-registered Occupational Therapist with a Full Class C Drivers License
- You bring proven experience in community settings and have a passion for delivering compassionate care to our vulnerable populations.
- Confident in applying clinical reasoning and exercising sound professional judgment in a fast-paced, multidisciplinary environment
- A natural communicator who thrives in a team, building strong relationships with patients, families, and colleagues alike
- Committed to continuous improvement, with a demonstrated history of contributing to quality initiatives and evidence-based practice
