Occupational Therapist – Reablement at Home

Liquid Personnel is recruiting an Occupational Therapist – Reablement at Home to join its client’s Reablement at Home team, based in Hackney.

This role sits within a therapy‑led reablement pilot service, designed to inform future recommissioning of wider homecare services. The focus is on supporting adults to regain independence at home following hospital discharge.

What will your responsibilities be? 

  • Deliver short‑term reablement interventions (up to 6 weeks)
  • Support service users to regain or retain daily living skills
  • Work from Reablement Therapy Plans provided at referral
  • Promote strengths‑based, outcome‑focused practice
  • Use community resources beyond personal care
  • Gradually reduce support as goals are achieved

Service Aims

  • Enable independence and autonomy
  • Reduce or avoid dependency on long‑term services
  • Deliver care closer to home and at the right time 

Qualifications and Experience: 

To be successful in this role you must have;  

  • HCPC‑registered Occupational Therapist
  • Experience in reablement, discharge or community OT
  • Strong person‑centred assessment skills
