Occupational Therapist - Locality MDT (Discharge to Assess)

Liquid Personnel is recruiting an Occupational Therapist for its client’s Locality MDT (Discharge to Assess) Team in Redbridge. The role is locality‑based and focuses on supporting Discharge to Assess referrals across multiple clusters, managing a mixed caseload of complex and standard Occupational Therapy cases.

What will your responsibilities be?

  • Manage approximately 4 new cases per week
  • Undertake holistic, person‑centred assessments and reviews
  • Lead on specialist seating, adaptations and manual handling equipment
  • Recommend, arrange and review complex equipment provision
  • Provide training to ensure safe and effective equipment use
  • Manage complex cases autonomously and effectively
  • Work collaboratively within a multidisciplinary team

Qualifications & Experience:

To be successful in this role you must have, 

  • HCPC‑registered Occupational Therapist
  • Extensive experience with specialist equipment, adaptations and manual handling
  • Strong understanding of adult social care and housing legislation
  • Confident autonomous practitioner with MDT experience
