Liquid Personnel is recruiting Occupational Therapist for its client's integrated Triage Team at Bexley.
The Bexley Integrated Triage Team is a multi‑disciplinary service providing initial triage, assessment, and response for adults with health and social care needs across primary and secondary care settings. They offer holistic, person‑centred assessments both in the home and over the telephone, ensuring residents receive the right intervention at the right time.
This is an excellent opportunity for an OT confident in community work, holistic assessment, rehabilitation planning, and supporting adults with a range of physical and mental health needs.
What will your responsibilities be?
- Deliver professional, personalised OT assessments for adults and older adults with physical disabilities and dual physical/mental health needs.
- Provide holistic assessments at home and via telephone triage.
- Recommend rehabilitative, preventative, and independence‑focused solutions to improve safety and quality of life.
- Lead on Access / Home visits and functional assessments across the borough.
- Contribute to shaping and improving the OT service through continuous evaluation and professional feedback.
- Work collaboratively with:
- Nurses
- Social Workers
- Rehabilitation Assistants
- Physiotherapists
- Doctors
- Volunteers and wider MDT colleagues
- Manage a varied caseload, ensuring timely and defensible clinical decision‑making.
- Supervise and support junior staff as required.
- Attend MDT and family meetings, offering OT expertise to guide joint planning.
- Travel independently across the borough to complete home‑based assessments (driver not mandatory unless required by service, but ability to travel is essential).
Qualifications & Experience:
To be successful in this role you must have,
- Degree / Diploma in Occupational Therapy.
- Experience working with adults and older adults with physical disabilities and dual diagnosis.
- Strong holistic assessment and rehabilitation planning skills.
- Ability to work autonomously and collaboratively in a fast‑moving MDT environment.
- Excellent communication skills across clinical, social care, and multidisciplinary settings.
- Confidence managing a community caseload and prioritising effectively.
- Ability to travel independently to home visits across the borough.
