Liquid Personnel is seeking an experienced Occupational Therapist to support the delivery of Hospital Discharge / Home First at Trowbridge.
This role has become available due to increased service demand following the successful implementation of the Home First contract. The Occupational Therapist will work in partnership with acute hospital teams to support safe and timely discharges into the Reablement Service.
The ideal candidate will have strong experience working in fast‑paced hospital environments and be confident managing high referral volumes and complex cases.
