Summary

The Nursing Intake Specialist is responsible for initiating the first contact between the nursing team and patient. The specialist will work with Nursing Management and Nursing supervisors to coordinate new patient initial visits and complete post-infusion assessments. Participates in Quality Improvement Projects as directed. The specialist will attend to the patient’s needs with utmost patience, care, compassion and understanding.

Location: Plano, Texas
Schedule: Monday – Friday; 8:30am – 5:00pm (100% On-Site)
Pay Range: $24.00 - $38.00/hr (DOE)

Essential Duties and responsibilities

Responsibilities

  • Conduct a phone interview with new patients upon pharmacy initiation to schedule.

  • Introduce the patient to appropriate supervisor, providing their name and phone number.

  • Provide a brief overview of home infusion visits.

  • Gather details regarding patient scheduling needs (i.e., patient preferred schedule dates/times, etc.)

  • Responsible for conducting post-infusion assessments upon patients completed initial first visit to assess patient outcomes.

  • Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.

  • Documents current clinical data on the patient’s profile as appropriate.

  • Collaborates with other referral stages, such as Intake Prep, Patient Care Coordination, Pharmacist Verification to ensure complete patient care.

  • Conducts disease specific clinical assessments, as applicable.

  • Maintains continuity among teams by documenting and communicating actions, irregularities, and continuing needs.

  • Maintains patient confidence and protects operations by keeping information confidential.

  • Provides high quality services to the home-care patient and the home-care referral source.    

  • Consistently represents the company in a professional manner. 

  • Maintains effective working relationship and cooperate with all personnel in the Company.

  • Adheres to the Company’s compliance requirements as stated in the policy and procedure manual and all other related policies.

  • Performs other duties and responsibilities as assigned.

Qualification Requirements

Basic Qualifications

  • Licensed practical/vocational nurse with current state licensure

  • Work experience as a professional nurse, preferably in a like work environment

  • Must possess the ability to make independent decisions, when necessary

  • Must possess the ability to deal tactfully with a team of inter-disciplinary professionals, residents, family members, visitors, government agencies, and the public.

  • Must be knowledgeable of nursing and medical practices and procedures, as well as state, federal and regulatory requirements that pertain to long-term care.

  • Must be able to interpret and implement the programs, goals, objectives, policies, and procedures of

  • Basic level skill in Microsoft Excel (for example: using SUM function, setting borders, setting column width, inserting charts, using text wrap, sorting, setting headers and footers and/or print scaling)

  • Basic level skill in Microsoft PowerPoint (for example: applying a theme, formatting character spacing, inserting a picture, changing slide layout and theme colors, adding transitions, customizing slide numbers, changing chart style and/or formatting font)

  • Basic level skill in Microsoft Word (for example: inserting headers, page breaks, page numbers and tables and/or adjusting table columns)

Education and/or Experience

  • Active Licensed practical/vocational nurse

  • Two years’ experience working as a nurse in a Specialty Pharmacy (preferred)

  • Prior experience in home infusion, specialty pharmacy, or complex therapy intake (preferred)

  • Strong background in insurance verification, benefits investigation, or PA coordination (preferred)

  • Internal equity across Texas-based Intake, PCC, and Patient Access roles (preferred)

  • Market competition within the Plano/Dallas healthcare sector (preferred)
