As a Nursing Admission Specialist, you will initiate and/or resume home care for patients of all ages and health backgrounds in in the east region, including the Eastern Cuyahoga, Lake, Geauga and Ashtabula Counties, offering comprehensive assessments, care plans and ongoing support to meet their unique needs. Your efforts help them stay independent and confident when managing their own care, ultimately reducing hospital readmissions and poor outcomes. This position offers rewarding experiences and multiple resources for enhancing your nursing skills, education and professional development.
A caregiver in this position works days from 8:00am to 5:00pm, with one on-call shift per week. The primary on-call shift is nights from 5:00pm to 8:00am and the secondary on-call shift is 5:00pm to 9:00pm. 1 weekend every 4 weeks and 1 winter/1 summer holiday are required.

Physical Requirements:
Must be ambulatory and able to drive to patients homes and potential customers, able to climb stairs, pull, push, and facilitate movement of the patient.
Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.
