Your Role & Responsibilities
You will provide comprehensive care to seniors following clinical standards.
You will collaborate daily with your care team to ensure coordinated, high-quality care.
You will manage referrals, hospital/SNF coordination, DME, and home health services.
You will document care with support from quality-based coders.
You will participate in on-call rotation and support growth plans at your center.
You will have experience providing value-based care.
What We're Looking For
Required Qualifications
a Bachelor's degree
Masters in Science of Nursing /PA
Active, unrestricted Nurse Practitioner/PA license in the appropriate state
We consider this role patient facing and make it part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, we will require you to undergo TB screening.
Must be passionate about contributing to an organization focused on improving consumer experiences
Must be willing to participate in limited on-call coverage by phone
You will report to the Associate Medical Director in market.
This role is onsite in our Richmond Road clinic.
