RYZ Labs is hiring a Novice LPN - Private Duty to provide services to patients/clients in their places of residence.
Responsibilities:
- Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or contracted services
- Demonstrates on each assignment the provision of skilled nursing care rendered in accordance with prescribed orders
- Identifies and documents changes in the patient’s both physiologic and psychosocial condition
Notifies Director/Supervisor if applicable on the following;
- Physician notification to obtain necessary orders for intervention(s) per state regulations
-Changes in patient/client condition
-Adheres to Maxim's policies and procedures and applicable protocols
- Identifies and prioritizes actions based upon patient/client care requirements
-Assures patient safety at all times and is the recipient of services provided
- Verifies that documentation reflects patient/client collaboration, participation in plan of care, expected patient/client outcomes, and selected nursing interventions
- Performs interventions, treatments, and medication administration, according to identified priorities, plan of care, facility policies, and patient/client care outcome standards as applicable.
- Monitors and documents the effectiveness of interventions, treatments, and medications
- Evaluates patient/client and provides a safe, comfortable, and therapeutic patient/client environment
- Monitors the safety and effectiveness of the environment and equipment, including universal precautions
- Participates in activities to facilitate good interpersonal communications
- Communicates observations and significant changes in patient/client status to the Director of Clinical - - Services and/or Clinical Supervisor /Case Manager or licensed designee
- Documents all physician collaboration/coordination of care in the medical record and
communicates this information as needed
- Attends case conferences, team meetings, and education in-services as required
Supports professional practice by observing the following;
- Nurse Practice Act
- Standards of Nursing Practice
- Standards of Patient Care
- Standards of Accrediting Body (ACHC, Joint Commission, etc.)
- State and Local Laws
- Patient Bill of Rights
- Maxim Healthcare, Nursing, and Client’s Hospital Policies
- Performs other duties as assigned
Requirements:
-Graduate of an accredited school of nursing
- License Required
- 3.0 GPA preferred
- Complies with accepted professional standards and practice
- Current Hands-On BLS card
- PPD or chest X-ray, if applicable
- Successful completion of the Maxim Homecare Novice Nurse Program orientation and training
- Proficiency in the English language is required
- Excellent communication skills
- Technological proficiency, including mobile devices for documentation
