Allina Health is seeking a Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), or Licensed Professional Clinical Counselor (LPCC) to join our Adult Partial Hospitalization Program (PHP) team in Faribault, MN. This role focuses on providing evidence-based therapy, diagnostic assessments, and individualized treatment planning for adults in a structured outpatient treatment setting.Key Position Details:
As a Mental Health Licensed Therapist (LMFT, LICSW or LPCC), you’ll perform mental health diagnostic assessments and provide individual, family, and group therapy within established programs. Responsibilities include developing treatment plans, coordinating care with community providers, and supporting patients through their course of care. For Intermediate Care Programs, practice aligns with CMS, MN DHS Manuals, and MN §245I standards.

Job Description:
Key Responsibilities
Perform mental health diagnostic assessments per regulatory standards.
Develop and implement individualized treatment plans with patient and family input.
Provide individual, group, and family therapy aligned with clinical goals.
Coordinate care with community providers and agencies; initiate referrals as needed.
Ensure compliance with CMS, MN DHS, and MN §245I documentation requirements.
Offer clinical supervision for Mental Health Practitioners when applicable.
Required Qualifications
Master’s degree in psychology, family therapy, social work, or counseling-related field.
MN licensure as LMFT, LICSW, LPCC, or Licensed Psychologist upon hire.
MN DHS Mental Health Professional qualifications per MN §245I.04.
Preferred: 2+ years of clinical experience.
Current BLS certification (American Heart Association, Red Cross, or Allina in-house within 180 days).
Suicide prevention training within 90 days of hire.
Physical Demands

Pay Range
Pay Range: $34.83 to $47.74 per hourThe pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable).
