Job Title: Medical Records Clerk
Department: Health Information Management
Location: 330 Monroe Avenue Rochester, NY 14607
Hours Per Week: 40 hours per week
Schedule: 08:30am-5:00pm, no weekends
Position Summary:
Responsible for initiating, maintaining, delivering, and retrieving all patient medical records.
Key Responsibilities:
Maintain confidentiality of patient information.
Responsible for accurate and thorough maintenance of the medical record.
Medical Records only: Consistently utilizes effective oral and written professional communication skills with all customers. Requires personal contact with customers, ie.hospital staff, patients, physicians.
Medical Records only: Responsible for answering telephone, fax machine, and use of other office equipment. Provide general clerical and receptionists support the department.
Medical Records only: Responsible for maintenance of various databases (CRIS,SMS,IMA).
Minimum Qualifications:
High School Diploma or equivalent.
Must be able to alphabetize materials and be able to identify and distinguish colors.
Keyboarding skills and familiar with Windows environment

RESPONSIBILITIES
· Daily tracking and follow up on MD orders to ensure signature obtained within regulatory timeframes including phone/fax contact with offices and logging of such actions in KOVIS.
· Escalation of barriers to obtaining signed home care orders in a timely manner to proper department as necessary.
· Reviews incoming orders both via fax and mail, for proper signature and date. Initiate follow up for inadequacies as necessary.
· Processes requests for data entry of new MDs into Netsmart after researching PECOS, OPRA, NPI and MD License.
