The Medical Director, Population Health will report to the Senior Medical Director, Population Health - Complex Patient Care and will play a pivotal role in shaping and guiding our clinical programs for patients who are at greatest risk for poor outcomes due to chronic conditions, behavioral health challenges, and social barriers to care.
 

This clinician leader will design, implement, and continually improve the systems and strategies that help Oak Street clinicians care for our most complex patients—those whose health is influenced by overlapping medical, psychological, and social factors. The role spans clinical innovation, data-informed program design, and collaborative leadership to ensure every complex patient receives coordinated, compassionate, and effective care.
 

The Medical Director will work cross-functionally with operations, analytics, technology, and regional care teams to create an integrated approach that improves outcomes, prevents avoidable hospitalizations, and helps patients achieve their fullest health potential.

Core Responsibilities
 

Lead Clinical Strategy for Complex Care

  • Provide clinical direction for Oak Street programs focused on patients with significant medical complexity, behavioral health conditions, or unmet social needs.

  • Develop and refine methods for identifying and stratifying complex patients using data analytics, clinical insight, and social risk assessment.

  • Guide the design and rollout of interventions that address the full spectrum of patient needs, from medical management to social supports and caregiver engagement.

  • Ensure that care models are evidence-based, equitable, and aligned with Oak Street’s mission to deliver high-quality, outcomes-driven primary care.
     

Advance Integrated and Coordinated Care

  • Strengthen communication and collaboration across primary care, specialty care, behavioral health, and community-based services.

  • Build processes that support optimal transitions of care—particularly from hospital to home—to improve health outcomes and decrease readmissions.

  • Partner with care management, pharmacy, and social work teams to ensure each complex patient has a personalized and well-coordinated care plan.
     

Drive Organizational Strategy and Innovation

  • Collaborate with Population Health and Clinical leaders to develop the long-term vision for complex patient management across the enterprise.

  • Work closely with Oak Street’s Product and Technology teams to enhance digital tools that support clinical decision-making and care team workflows.

  • Use data to evaluate program performance, identify improvement opportunities, and drive rapid-cycle innovation.

  • Serve as a thought leader and internal consultant on complex care strategy, representing Oak Street’s approach within and beyond the organization.
     

Foster Culture and Leadership Excellence

  • Model Oak Street’s culture of optimism, teamwork, and continuous improvement.

  • Mentor clinicians and team members to build expertise in caring for patients with complex needs.

  • Champion a patient-first mindset that values compassion, accountability, and clinical excellence.
     

Clinical Practice

  • Maintain a direct connection to patient care by practicing in an Oak Street Health Center 2 days per week, ensuring firsthand insight into patient and team experiences.

What We’re Looking For
 

Qualifications and Experience:

  • Authorization to work in the U.S.

  • M.D., D.O., APN, or PA with active clinical license and board certification in Internal Medicine, Family Medicine, Preventive Medicine, or Geriatrics.

  • Demonstrated experience leading programs or teams focused on complex or high-risk patient populations.

  • Strong familiarity with managed care, value-based reimbursement, and population health management.

  • Proven ability to analyze and apply data to guide program improvement and measure clinical outcomes.

  • Exceptional collaboration and communication skills, with the ability to influence across disciplines and functions.

  • Advanced training or degrees (e.g., Fellowship, MPH, MBA) and/or experience in management consulting preferred.

Pay Range

The typical pay range for this role is:

$174,070.00 - $374,920.00


This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.  This position also includes an award target in the company’s equity award program.
