Job Description

Summary

This position provides clinical leadership and direction to the Case Management team. The manager works to create a work environment that engages the staff in the attainment of department and organizational goals. The manager must be knowledgeable in all areas of Case Management including utilization management, care coordination, and

discharge planning. The incumbent collaborates with members of the

multidisciplinary team in order to facilitate patient centered care. The incumbent participates in the coordination, development, execution, monitoring, and evaluation of all Case Management activities.

Area Specific Job Accountabilities

•    Works with Case Management senior leadership in the development of objectives and operational practices that ensure critical success factors

involving utilization management and care coordination are achieved.
•    Coordinates the daily activities and provides supervision of the Case Management Team.
•    Recognizes and reports variance of workflow and daily operations.
•    Supervises the Case Management Associates who participate in the

utilization management process.
•    Represents and promotes the hospital through relationships with other members of the patient care team.
•    Maintains a high degree of awareness and knowledge of the hospital's programs, services, and staff.
•    Participates in the recruitment, hiring, and coaching of Case Management staff. Provides ongoing staff development, including the

annual performance appraisal/development process.
•    Works in collaboration with the Case Management Specialist in new

employee orientation for the department to assure that all newly hired staff are sufficiently prepared to assume their job responsibilities.
•    Performs daily rounds with the case management team to facilitate proactive problem-solving.
•    Serves as a resource person, clinical expert and role model. Meets

regularly with direct reports to provide clinical supervision and to facilitate professional growth.
•    Performs job functions adhering to service principles with customer

service focus of innovation, service excellence and teamwork to provide the highest quality care and service to our patients, families, co-workers and others.

Knowledge, Skills and Abilities

·         Graduate of accredited school of nursing. BSN or higher required.

·         Currently licensed as a Registered Nurse within the State of Illinois.

·         Minimum 3 years RN experience with at least 1 year Case Management experience.

·         Previous supervisory experience preferred.

·         Certification in Case Management required (CCM) or acquire within one year of hire.

·         Working knowledge of Midas+ and InterQual Evidenced Based Criteria. Certified InterQual Instructor preferred.

·         Knowledge of payer reimbursement methodologies.

·         Demonstrates excellent leadership skills, ability to lead a team;

excellent written and verbal communication skills and excellent organizational skills.

·         Working knowledge of community resources and/or how to access these systems for appropriate discharge planning modalities.

·         Ability to mentor and coach to help staff achieve professional and organizational goals.

·         Self-directed learner and professional. Enthusiastically seeks ways to improve the Case Management process.

Pay Range

$99,840.00-$164,736.00 Salary

The pay range for this job indicates minimum and maximum targets for the position.
