Position Summary

** Candidates must reside in West Virginia**


- Involved in initiative planning, ongoing client relationship management and satisfaction for assigned initiatives that impact the delivery of products and services to the internal and external environment
- Collaborates with external vendors, external consultants, consulting team members and/or matrixed staff as required in support of initiatives.
- Accountable for transferring knowledge, best practices (external and internal), methodology and tools to client organizations.
- Participates as a team member, may be responsible for concurrent initiatives or act as team lead for less complex projects based on resource requirements.
- Supports holistic solutions and/or key strategic plans. Proactively identifies opportunities for improvement within assigned initiatives
- Implement and scale the overall strategy to address SDoH for members and communities across state
- Operationalize SDoH product in local markets
- Hire, train, and manage field staff in assigned markets
- Establish and build key relationships with community-based organizations to execute SDoH strategy
- Identify opportunities to develop and pilot new SDoH initiatives
- Gather input and maintain proactive communication across business units to coordinate SDoH function across the market



Required Qualifications

- Must live in West Virginia

- 5 + years’ experience with population health, care coordination, and/or community-based programs
- 3 + years in a leadership role
- Experience leading in a matrix environment
- Prior Managed Care Experience
- Ability to travel up to 25% of the time within the assigned region, to multiple locations

- Must have access to reliable transportation, valid/active driver's license, and proof of automobile insurance


Preferred Qualifications

- Master’s Degree or higher in health administration, social services, public health, business administration, psychology or closely related field
- Excellent interpersonal and strong communication skills
- Demonstrates good oral and written presentation skills
- Strong organizational skills, including the ability to prioritize, multi-task, and manage workload to meet specific timeframes and deadlines

Pay Range

The typical pay range for this role is:

$54,300.00 - $119,340.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
