Collaborates with interdisciplinary team in the delivery of patient care, under the direction of a Registered Nurse, utilizing the nursing process.

Job Description

We are seeking a skilled LVN with outpatient experience and strong EPIC proficiency who thrives in a fast-paced clinical setting. This role plays a critical part in supporting providers, ensuring clinic efficiency, and delivering high-quality patient-centered care.
Key Responsibilities & Strengths:

  • Accurate and timely patient triage, including assessment of walk-ins and urgent concerns
  • Performance and documentation of point-of-care testing with appropriate follow-up
  • Effective management of patient assistance programs
  • Oversight of clinic supply inventory to ensure operational readiness-
  • Processing medication refills in compliance with clinical protocols
  • Prompt response to patient and staff emergencies with appropriate escalation
  • Delivery of comprehensive patient education to reinforce care plans
  • Completion and coordination of FMLA and related clinical documentation

The ideal candidate is organized, proactive, and comfortable balancing clinical and administrative responsibilities. Critical thinking, compassion, autonomous, and efficient communication skills and the ability to prioritize in a high-volume outpatient environment are essential.

Job Description

MINIMUM QUALIFICATIONS

Education:  Graduate from an accredited school of vocational nursing or licensed practical nursing

Licenses/Certifications: 

  • Current State of Texas license or temporary/compact license to practice as a Licensed Vocational Nurse (LVN);

  • Certified in Basic Life Support

  • Additional certifications and language skills may be required based on discipline and /or nursing unit requirements

Experience / Knowledge / Skills:

  • Minimum of (3) three years prior LVN experience preferably in acute care setting

PRINCIPAL ACCOUNTABILITIES

  • Delivers safe and appropriate care to patients, according to the nursing skills inventory and the age and needs of patient/population served, within the scope of licensure.

  • The LVN collaborates with the interdisciplinary team in the delivery of patient care; supporting and collaborating with the Registered Nurse in providing direction to the healthcare team.

  • Participates in a systemic, interdisciplinary, and ongoing evaluation of programs, process improvement and desired client-centered outcomes.

  • Considers the diverse needs of each patient through the entire continuum of care, providing and ensuring the highest level of service according to established service standards, optimizing the patient experience.

  • Adheres to all regulatory and Texas Board of Nursing standards. Performs other duties as assigned, within the scope of practice and clinical competency.

  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.

  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
