CC015041 Arkansas Children's Pediatrics - LRThe Patient Navigator case manages and coordinates aspects of care for treatment of the Complex Care Program by working with the patient and family to provide education and collaboratively establish a plan of care. Serves as a liaison among physicians, nurses, ancillary departments, patients, families, and office staff to ensure effective utilization of resources and coordination of care.
1. Conducts telephonic outreach to patients in need of health care services and educates them on the importance of receiving the needed service.
2. Monitors delivery of care, identifying progress toward desired outcomes, intervening to overcome barriers and deviations in the expected care plan, interacting with departments to negotiate and expedite scheduling and completion of tests, procedures and consults.
3. Assists patients with DME, prescriptions, prior authorizations, and other communications related to the patient/family needs.
4. Documents the outcome of the outreach efforts in the patients EMR record and on tracking spreadsheet.
5. Contributes to patient plan of care based on clinical, educational, psychosocial, and cultural needs while recognizing when to resource other health team members.
6. Plans, prioritizes and organizes patient care tasks to ensure time and resource utilization.
7. Completes routine patient care tasks and medication administration in a sage, efficient and timely manner but recognizing and intervening in emergency situations.
8. Individualizes nursing interventions to meet identified needs and modifies interventions when the needs of the patient or family change.
9. Reassesses and modifies patient plan of care in collaboration with the registered nurse.
10. Performs other duties as assigned.
