Job Description Summary:
This is a full time, day shift, Monday through Friday LPN position. There are no weekends and no holidays. Working with nursing staff and providers to deliver post-discharge care exclusively for trauma patients. Responsibilities include rooming patients in the clinic and assisting with incoming telephone calls related to AVS questions, home health, DME, prescriptions, referrals, scheduling, follow-up needs, along with other clinic tasks. The role also supports completion of FMLA and Workers’ Compensation paperwork, manages nursing charges, supply orders/auditing, partners with the multidisciplinary and support teams to address the mental health needs of patients recovering from traumatic injuries such as gunshot wounds, stabbings, falls, motor vehicle crashes, etc. We are not a primary care provider. We are an ambulatory clinic dedicated to supporting patients through recovery from traumatic injuries. Patients may be scheduled for one or multiple clinic visits to fully address their needs. They work collaboratively with the inpatient team to ensure patients are scheduled for their clinic. Daily chart reviews for upcoming appointments to prepare for future visits. The clinic team consists of LPN, APP, Social Workers, Community Health Worker, and Front Desk Representative.Responsibilities And Duties:
25% Performs initial, ongoing and functional health status assessment as applicable to the population and/or individual. Collects and documents initial and ongoing subjective and objective data from patients, significant others, and communities. Reviews data obtained by other members of health care team and incorporates into assessment. Identifies and communicates assessment findings, both initial and ongoing, to appropriate people via referrals, rounds, huddles, reports and care conference, etc. Uses assessment data to identify pertinent nursing diagnoses and collaborative problems. Collaborates assessment findings with providers and members of healthcare team as appropriate. Uses scientific knowledge, Critical thinking skills, technical skills, and effective interpersonal and teaching skills to provide nursing care.Minimum Qualifications:
