MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Current Licensed Practical Nurse license issued by the state in which services will be provided or current multi-state Licensed Practical Nurse license through the enhanced Nurse Licensure Compact (eNLC).
2. Completion of an accredited LPN Program
3. State criminal background check and Federal (if applicable), as required for regulated areas.
4. Certification in Basic Life Support within 30 days of hire date.
5. Must possess a valid and current driver’s license.
EXPERIENCE:
1. One (1) year of clinical nursing experience.
PREFERRED QUALIFICATIONS:
EXPERIENCE:
1. Specialized LPN experience.
2. Two (2) years of experience related to intravenous administration of medications.
3. Two (2) years of Home Health experience.
4. One (1) year experience in care management.
5. Sales and Customer Service experience.

1. Supports communication and coordination of discharge planning to ensure timely care for new patient referrals amongst pharmacy staff, home care nursing staff, care management, providers, and other clinical support staff.
2. Assists in the development and monitoring of workflows, policies, procedures, projects, and compliance within home infusion services under supervision.
3. Facilitates assessment of home infusion related issues, providing basic interventions or referrals as appropriate and within scope of practice.
4. Demonstrates knowledge of specialty & core therapies and the home infusion referral, as well as payor qualifying criteria relevant to assigned responsibilities.
5. Reports complaints and issues to supervisors/management as applicable for further evaluation and resolution.
6. Maintains positive relationships with referral sources, care management, pharmacy staff, and home care nursing staff.
7. Supports implementation and delivery of patient care plans and home infusion clinical programs, ensuring
8. appropriate follow-up and delivery of care as per the established plan.
9. Accurately documents the coordination & transition of clinical care and patient education efforts following established protocols.
10. Participates in training and cross-training initiatives to promote team efficiency and productivity.
11. Provides consultation and patient/caregiver education as needed to ensure care coordination among the care team.
12. Supports identification, development, and monitoring of performance and quality improvement initiatives.
13. Assists with obtaining prior authorizations for patient services, working with healthcare providers and insurance companies as needed.
