Overview:
The CDC Foundation is working with CDC and state and local authorities to provide surge staff to support surveillance, prevention and response activities within the Overdose Data to Action (OD2A) program. The CDC Foundation seeks candidates for a Linkage to Care Technical Assistance Coordinator. Under general supervision, the Linkage to Care Coordinator will provide education, outreach, and linkage to care for people with opioid use disorder, substance use disorder or co-occurring disorders. The Linkage to Care Technical Assistance Coordinator will identify opportunities and barriers to accessing care and work with existing linkage to care staff to provide guidance to overcome challenges, create effective processes and increase collaboration.


Minimum Qualifications:
· Minimum of three (3) years of full-time professional work experience with linkage to care, care coordination or case management.
· Bachelor’s degree in Public Health, Social Sciences, or related field; Master’s degree preferred.
· Demonstrated knowledge in overdose prevention, substance use disorder treatment, community-based overdose prevention, recovery, and/or drug policy
· Experience utilizing trauma-informed and strengths-based approaches.
· Highly motivated and organized with the ability to work well in teams and independently.
· Strong written and oral communication skills, with ability to be efficient and consistently deliver high-quality work under time constraints.
· Ability to coordinate and collaborate with diverse stakeholders.
· Knowledge of community resources preferred.
· Demonstrated ability to work well independently and within teams
· Experience working in a virtual environment with remote partners and teams
· Proficiency in Microsoft Excel, Word, PowerPoint, Teams and Zoom

Responsibilities:
· Provide assistance with outreach, engagement, referrals, warm hand offs, and care coordination services for individuals with substance use disorder and co-occurring disorders.
· Advise on evidence-based and accurate substance use disorder education to individuals, groups, and organizations.
· Develop networks with external stakeholders and service providers to increase access to community-based resources.
· Provide assistance with assessing and managing referral processes to address necessary challenges for patients and providers.
· Coordinate with community partners on access to evidence-based support and programs, including additional non-medical needs of individuals (for example, housing, employment, insurance, food, internet, etc.).
· Support people engaged in care.
· Assist with conducting outreach to engage or re-engage individuals in care.
· Participate in evaluation and quality assurance activities.
· Manage inter-agency coordination by establishing and maintaining structured workflows between partner agencies; working directly with Peer Navigators to support and streamline client assistance efforts.

Roles and responsibilities listed above may be expanded upon or updated to match priorities and needs, once written approval is received by both the CDC Foundation and Douglas County Health Department to best support Omaha, Nebraska in their public health programming.
