Community Support Team Leader- provides direct therapeutic services to adults with MH/SU needs in the community, supervises team of dedicated staff (QP, LCAS, PSS, CADC), navigates community systems to address social determinants of health, works collaboratively with multidisciplinary team; and provides crisis services when needed.Location: Caldwell and Burke Counties
Schedule: Flexible based on needs of individuals served; 40 hours per week; some on call rotation required
Salary: Dependent on licensure and experience; $62,000-$66,000 Provisionally licensed; $67,000-$72,000 Fully licensed
Job Responsibilities:
Providing therapeutic services including assessments, crisis intervention, and individual therapy to adults in the community.
Assisting people with learning to live independently in their own homes.
Leading a team of behavioral health specialists, providing management support, supervision, and other related tasks to insure quality care of caseload of up to 48 individuals.
Serving individuals with wide variety of mental health and substance use diagnoses.
Facilitating relationships and serving as a link between the company, parents, guardians, local agencies and the community.
Supporting individuals with connections to services and referral for holistic treatment.
Working closely with other clinical/professional staff to maintain communication and providing feedback, standardizing procedures and expediting PCP implementation.
Ability to perform tasks on the computer including working with Microsoft 360, Excel, Word and other basic programs.
Valid driver’s license, auto insurance and reliable transportation
Ability to pass a drug screen and background check
Ability to lift between 20 and 50 pounds and meet the physical requirements of performing CPR and therapeutic holds.
