Sentara Health is seeking to hire a qualified individual to join our team as a Licensed Practical Nurse.
Overview:
The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being.
Anticipated Status/Shift:
Full Time (40 hours/week)
Evening Shift (4:30PM - 12:30AM)
Occasional weekends
Position Location:
Sentara Hospice Services - Suffolk, VA (travel within a 40-mile radius)
Pay Range: $22.39 - $37.32/hour
We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
Education
Trade School Graduate for Nursing (Required)
Certification/Licensure
LPN License in state of practice (VA) (Required)
BLS (Required within 90 days)
Experience
2 years' experience (Required)
Serve as patient advocate in the prevention of illness or disease (Preferred)
Provide care and treatment for patients and clinical support and teaching for patient and family members (Preferred)
Customer service, testing, medication administration and the implementation of nursing interventions and care plans (Preferred)
Support a holistic approach to patient care (Preferred)
