Job Summary: The incumbent will performs telehealth imaging including but not limited to teleretinal screening, teledermatology tele-wound care imaging and other telehealth programming as technology becomes available and as identified and referred by the ordering/referring provider, guided by local policy, and standing orders the LPN TCT can initiate consultations for patients on behalf of the primary care or specialty provider who retains responsibility for patient care.

Major Duties:

Provides technical support and training Provides completion of business processes, administrative support, and other program support duties as needed Demonstrates active collaboration and communication with the patient, facility telehealth coordinator and patient aligned care team (pact) involved in providing care Performs telehealth imaging and other telehealth programming as technology becomes available and as identified and referred by the primary care provider and other providers. Provides advanced clinical technology information to patients staff and providers regarding telehealth education and training processes, assist the remote and local provider, employees, and patients as needed. Has regular contact with the facility coordinator to work out any process issues equipment needs problems data collection and any other logistical issues opens the telehealth patient room readies the equipment for examination training education access and establishes the online conferencing equipment or other connections Coordinates the information between facilities to expedite patient clinical care through telehealth modalities such as SFT and CVT. Provide access to promote and ensure continuity of care. The LPN/TCT Is the point of contact for both outpatient and inpatient referrals requiring telehealth services synchronizes telehealth modalities with medical staff, patients, and facilities telehealth services and emergency services. Assists in developing programs and processes to ensure telehealth modalities are timely efficient and appropriate to the needs of the patient schedules confirms and coordinates telehealth services of inpatient and outpatient clinics, also from VISN to VISN, facility to facility CBOC to CBOC and private facilities including nursing homes hospitals and clinics Maintains daily telehealth schedule to assist in scheduling rooms patients technology and function as a telepresenter. Identifies and recommends ways of eliminating, combining, simplifying, or improving procedures and processes of telehealth program All other duties as assigned.
