Role Description:
The Population Health Specialist (PH Specialist) is a critical member of the regional leadership team. They partner closely with the Population Health Director (PHD) to execute on critical population health initiatives, focusing on accurate and compliant documentation, as well as performance in Oak Street’s Quality Program at the regional-level.
Core Responsibilities:
Support the development and execution of regional population health initiatives, owning small programs and initiatives as directed by the PHD
Work side-by-side with Medical Directors, Regional Vice President, QRN, OP, providers, and care team members
Support workflow optimization as it pertains to risk score accuracy and compliance, Quality Program, and all workflows related to the Care Report and Huddle Form
Support data analyses and reporting, including identifying opportunities via data focused on accurate clinical documentation and quality
Other duties, as assigned
What are we looking for?
We’re looking for motivated Sr CISs or Regional Leads with:
Expertise working with multiple technical systems
Strong understanding of key population health metrics
Ability to collaborate across multiple teams and engage in multiple perspectives to work toward a common goal
Proven experience and ability to develop, mentor, and empower a diverse team
Ability to succeed in a fast-paced, ever-changing environment
Self-starter with a high degree of drive, initiative, and follow through
Exceptional interpersonal, written and verbal communication skills
Commitment to company vision
Expert in accurate and specific documentation
Requirements:
At least 1 year of experience in a Sr CIS or Regional Lead role is required
Advanced computer skills required
Demonstrated ability to successfully lead process improvement projects
Ability to travel between clinic locations
US work authorization

The typical pay range for this role is:
$18.50 - $35.29This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
