40Lead Hospice Admission Nurse (RN)
Location: La Crosse, WI
FTE: 1.0 (80 hours per two weeks)
Pay: Starting pay of 38.01/hour, with additional compensation based on experience
Shift: 8-hour shifts, Monday-Friday 8am-4:30pm
Holiday: Rotating every 3rd holiday
Call Schedule: Back up on-call every 20th week for evenings and nights

We are seeking a Lead Hospice Admission Nurse (RN). As a Lead Hospice Admission Nurse, you will play a critical role in ensuring a smooth and efficient admission process for patients entering hospice care.
This position is primarily office-based and serves as the first clinical point of contact when a referral is received. The Lead Admission Nurse gathers and organizes all clinical information, prepares documentation, and ensures readiness for the admission nurse to visit the patient’s home. This role works closely with the Intake Coordinator, who manages non-clinical aspects of care, to provide a seamless experience for patients and families during the transition to hospice services.

What You Will Do
Receive and manage initial referral calls for hospice admissions.
Collect and review clinical information to prepare for patient admission.
Coordinate with admission nurses to ensure all clinical documentation and requirements are complete prior to home visits.
Collaborate with the Intake Coordinator to align clinical and non-clinical processes.
Serve as a resource for clinical questions during the intake process.
What You Need
Associate Degree in Nursing
Minimum of 1 year RN experience, hospice experience preferred
RN licensure to practice in Wisconsin upon hire
RN licensure to practice in Minnesota within 3 months of hire
Strong organizational and communication skills
Ability to work collaboratively in a fast-paced environment
