Education

  • Bachelor’s degree in nursing
  • Certification in Case Management

Experience / Qualifications

  • Minimum five (5) years advanced or specialized work experience in care management programs
  • 2 years progressive leadership and management experience

Position Summary

The Director of Case Management reports to the Vice President and Chief Nursing Officer and leads the overall operations of the Case Management department.

The Director of Case Management oversees a staff of case managers, social workers, and discharge planners responsible for patient care coordination, and staff who serve as clinical subject matter experts for clients implementing care management programs. Develops and implements case management programs, including intake or discharge planning, complex case management, and transition of care programs. Evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Develop partnerships with providers and clients to achieve quality and cost management objectives.

The Director ensures cost effective use of human and mental resources in delivering case management services.

  • Management of length of stay, observation patients, Outpatients in a Bed, care coordination and discharge planning.
  • Responsible for annual budget development, new program development, establishing department policies and procedures, department goals and objectives. 
  • Maintains quality assurance. 
  • Promotes and coordinates department Quality Improvement.
  • Coordinates activities with other departments to provide and maintain optimal standards of patient care. 
  • Manage process improvement initiatives, develops, and implements workflows.
  • Develops policies and procedures.
  • Develops, maintains, and oversee training materials and programs for case management staff training needs.
  • Compiles and reports case management data and other key metrics for internal and external reporting.
  • Responsibilities include the assurance that the staff adheres to the Jupiter Medical Center Compliance Program, to the rules and regulations of all local, state and federal guidelines, and to the standards of accrediting bodies.
  • Position works closely with the finance, decision support and the revenue cycle teams.
  • Effectively uses and role models current management techniques and theories, encouraging the decision-making skills of staff members and providing insight and leadership in complex situations.
  • Supervises the development of service policies, procedures, and operating guidelines.  Collaborates with other services within the division.  Ensures that information is disseminated to staff is monitored for effectiveness and changed as needed.
  • Understands and enforces all hospital and personnel policies and procedures.
  • Lead and/or serve on hospital and Utilization Management committees, teams, and work groups as required.
  • Performs other duties as assigned.

Physical Requirements

Requires sitting for long periods of time, also stooping, bending, standing and stretching. Must have manual dexterity for typing and computer data entry.  Able to work under a stressful environment, work independently, capable of making sound decisions, be detail oriented, alert and self motivated. Ability to push/pull up to 40lbs., carry and lift up to 20lbs.

  • Annual Joint Commission mandatory education requirements, in-service and health requirements including attendance at new employee orientation.
  • TB/PPD Surveillance Program.
  • Maintenance of required professional licensing and/or certification(s).
