Job Description

Job Title:  Imaging Nurse Navigator

Department: Imaging

Schedule: Monday-Friday, 8:30am - 5:00pm

Essential Job functions:

  • Navigates imaging breast patients across the continuum of care beginning with the diagnostic mammography through cancer survivorship, coordinating care with staff in Radiology, Surgery, Plastics, Oncology, Gynecology, Genetic Counseling, Internal medicine and Cognitive Therapy, and refers to community resources to facilitate timely diagnosis, treatment and survivorship. 
  • Navigates imaging patients across the continuum of care by coordinating advanced imaging appointments when needed related to incidental nodule findings, critical results, high risk screening assessments, etc. Works collaboratively across the continuum of care with staff in Radiology, Surgery, Pulmonology, Oncology, Gastroenterology, Orthopedics, etc.
  • Collaborates with Radiologists and imaging staff to ensure providers and patients are notified in a timely manner when an abnormality is identified.
  • Provides information on maintaining optimal health including instruction on breast self exam, smoking cessation, etc. and the importance of wellness exams to general population of patients.
  • Provides counseling for high-risk patients (breast and lung) and facilitates referral to surgical team, Pulmonology and Oncology.  Assists with further development of high-risk screening programs within Imaging.
  • Communicates with physicians and other members of the interdisciplinary team, within SMG, as well as with outside providers.
  • Facilitates scheduling of appointments with surgical and non-surgical staff to ensure timely referral and transition into specialty services.
  • Documents all patient visits and communication in both EMR and RIS systems. Manages and submits statistical data monthly.  
  • Attends committee meetings and provides quality driven data as well as metrics as requested. (Navigation team meetings, Interdisciplinary meetings, Breast Center staff meetings) providing input and suggestions on continuous quality and operational improvement.

General Job functions:

  • Participates in community outreach programs. (Breast Cancer Support Group Meetings, breast cancer patient classes, high risk breast, lung cancer prevention, evening lecture programs).
  • Coordinates the availability of patient education materials as well as being familiar with resources available in the community.

Education, Certification, Computer and Training Requirements:

  • Bachelor’s Degree desired.
  • Registered RN license.
  • BLS Certification required.
  • Minimum 3-5 years of experience.
  • Ability to communicate in English, both orally and in writing.
  • Ability to use Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred.
  • Ability to use Standard Office Technology in a Window based environment preferred.
  • Experience with EMR and RIS radiology System preferred.

Travel:

  • Ability to travel to satellite locations as needed

Pay Range: $87,600.00 - $109,600.00 Annual

The provided compensation range is based on industry standards and salary determinations will be made based on numerous factors including but not limited to years of experience and location of position.
