Summary of Duties

Assess individual applicants and clients in their care settings; develop, coordinate, and arrange a variety of services for elderly and disabled adults or for medical patients about to be discharged from the hospital, which will assure the continuity of care needed to enable them to maintain or achieve maximum physical, social, and emotional independence or rehabilitation. Work in collaboration with other, such as; discharge planners, adult protective services, case managers, community partners, and transition coordinators to facilitate and support diversion or transition efforts and other ODHS initiatives with the goal of diverting or transitioning institutionalized populations of people with complex medical and long term care needs into community bases settings with wrap around packages of services and supports.

Minimum Qualifications

A valid driver’s license and acceptable driving record are required for this position.

A Bachelor's degree in a Behavioral Science, Social Science, or a closely related field;

OR

A Bachelor's degree in any field and one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);

OR

An Associate's degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing);

OR

Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

Essential Attributes

We are looking for candidates with:

  • Experience with managing services for individuals

  • Experience interpreting and implementing policy and procedures

  • Experience preparing and maintaining written narrative documentation of actions taken

  • Experience interviewing to obtain information to evaluate the next steps to take in a process

Working Conditions

General office environment.   You will visit clients, hospitals, nursing homes, APD offices and other providers. A current driver’s license is required and an acceptable driving record.
