Your Opportunity

Common Ground Perth is an innovative housing and support service, delivered in partnership between Mission Australia and Nyoongar Outreach Services (NOS). Together, the partnership brings strong sector leadership, deep community capability and culturally informed practice to support people experiencing or at risk of homelessness in Western Australia.

Located in East Perth, the service follows a proven and recognised Housing First approach, providing permanent housing alongside intensive, on-site support. This is an on-site position based within a purpose-built facility comprising 112 self-contained single occupancy apartments, supported by communal spaces and a 24-hour support presence. Many residents have experienced chronic or persistent homelessness and require consistent, long-term support to sustain their tenancies and rebuild stability.

As a Housing Support Practitioner, you will contribute to the ongoing development and delivery of this service, working with residents from where they are at and for as long as support is needed. Through collaborative, trauma informed and recovery oriented practice, you will support people to maintain housing, strengthen wellbeing and build independence over time.

Your Key Responsibilities:

  • Deliver culturally safe, trauma-informed, holistic, and person-centred case management underpinned by Housing First Principles.
  • Conduct participant intake and safety/risk assessments using established frameworks and respond to referrals from internal and external services.
  • Support participants with practical assistance to live independently, including moving in to Common Ground, budgeting, passing property inspections, and completing paperwork.
  • Facilitate and participate in group activities that build community, capability, and capacity to thrive at Common Ground.
  • Offer tailored information, guidance, and referrals to support individuals to access relevant community services.
  • Engage collaboratively with internal teams and external partners through case coordination, advocacy efforts, and connection to appropriate support networks.
  • Maintain accurate and timely case notes and administrative records, ensuring compliance with reporting and quality standards.
  • Respond to trauma and crisis situations using Therapeutic Crisis Intervention (TCI) practices, supporting participants with complex needs including chronic homelessness.

Requirements for Success 

  • Minimum 2 years’ experience in case management within homelessness services, with a relevant qualification in Social Welfare, Social Science, or Human Services.
  • Demonstrated experience in case management and working with people with complex support needs.
  • Strong knowledge and experience working respectfully with Aboriginal and Torres Strait Islander peoples and protocols.
  • Trauma-Informed Care (TCI) and a recovery-oriented practice approach is highly preferred.
  • Excellent communication, decision-making, and organisational skills, with the ability to manage competing priorities.
  • Proven ability to follow policy and procedure, contribute ideas, and actively engage in service improvement.
  • Ability to work collaboratively across diverse teams and maintain professional boundaries in a fast-paced environment
  • First Aid Certificate is desired but not required.
  • Full driver licence with no restrictions

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
