The Salvation Army (Salvos) is one of the largest providers of homelessness services across the country. Our philosophy is that every person should be afforded dignity and respect and that no one should be without a safe, affordable, and secure home. We provide services and support to adults, and families with accompanying children in the form of accommodation, case management, financial assistance, community connection and referral services.

About the role 

As a Housing Case Manager, this is your opportunity to advance the issue of homelessness within the Wimmera South West and to provide assessment, support, and case management for clients who are homeless or at risk of homelessness. This role may include providing back-up support to other Homelessness programs operating from the Horsham office sites, and travel and outreach support to surrounding LGAs.

Reporting to the Wimmera Team Leader, this permanent part-time role offers 28.5 hours per week, located in Horsham, VIC. 

How you will make an impact  

  • Conduct comprehensive client intake assessments, provide outreach support across alternative sites, and ensure appropriate information and referral pathways are accessed.

  • Deliver practical and emotional support through ongoing engagement, and develop holistic, strength-based case plans that support clients to achieve their goals.

  • Create realistic housing and exit plans, conduct case closure meetings, and support clients to make informed and sustainable housing decisions.

  • Coordinate referrals to community services, day programs, training opportunities, job networks, and employment-related programs to enhance client independence.

  • Complete accurate case notes, reports, and data entry, participate in team meetings and CQI activities, and adhere to OHS requirements, organisational policies, and additional duties as required.

What you will bring 

  • A Tertiary qualification in Social Work or a related field and/or substantial
    experience 

  • Experience in working with vulnerable people 

  • A current VIC Drivers Licence and employee Working with Children’s Check

Conditions

Compensation is in accordance with Social, Community, Home Care and Disability Services Award Level 5
