Under the direct supervision of the Nursing Manager, the Hospice Nurse Case Manager is responsible for coordinating and delivering compassionate end-of-life care to patients and their families. This role involves assessing patient needs, developing individualized care plans, and collaborating with an interdisciplinary team to ensure the highest quality of hospice care.
Patient Assessment & Care Planning: Conduct comprehensive assessments and develop individualized care plans based on patient needs and goals of care ensuring optimal comfort and symptom management throughout the patient’s episode of care
Symptom Management & Pain Control: Administer medications, monitor symptoms, and adjust treatment plans to provide optimal relief
Interdisciplinary Collaboration: Work closely with physicians, social workers, chaplains, and other healthcare professionals to coordinate ongoing holistic care
Patient & Family Education: Provide support, resources and education on disease progression, hospice philosophy, and available support services. Support patients and families in making informed decisions about care and treatment options.
Emotional & Psychosocial Support: Provide compassionate care to patients and families, addressing emotional, spiritual, and psychological needs
Documentation & Compliance: Maintain accurate, compliant, and timely records of patient care, interventions, and progress while adhering to hospice regulations
Home & Facility Visits: Travel to patients’ homes, nursing facilities, and hospitals to provide direct care
Professional Development: Continuously seek opportunities for learning and skill enhancement in hospice and palliative care and care management
Required
Associate’s Degree or Equivalent in Nursing
2 years of relevant experience Acute care, RNC, ED experience
RN License in the State of Michigan
Michigan Drivers License
Preferred
Bachelor’s Degree or equivalent in Nursing
1 year of relevant experience Hospice
