Hospice Licensed Practical Nurse (LPN)

Monday through Friday 12:00p to 830p

What You’ll Do

As part of our home health team, you will provide skilled nursing under the supervision of a Registered Nurse—following your state licensure, professional standards, and Elara Caring’s mission-driven approach. Your day may include:

  • Delivering high-quality nursing care based on each patient’s personalized Plan of Care

  • Updating the RN Case Manager on patient changes, safety concerns, and progress

  • Supporting patient safety using family and community resources

  • Teaching patients and families simple, empowering self-care techniques

  • Helping prevent avoidable hospitalizations by watching for early signs of change

  • Documenting care, medications, and clinical findings accurately

  • Communicating positively with patients, families, physicians, and your team

  • Representing Elara Caring with professionalism, compassion, and integrity

Your work ensures that every patient receives “the right care, at the right time, in the right place.”

What You Bring

  • Graduate of an accredited school of nursing

  • Current LPN license (state-specific)

  • Minimum of 1 year of acute care experience; home health or hospice experience preferred

  • Strong clinical nursing skills

  • Excellent communication and interpersonal abilities

  • Effective time management—arriving to visits on schedule and completing documentation promptly

  • Ability to address third-party payor concerns and follow agency standards of care

  • Professional growth mindset and commitment to quality

  • Reliable transportation and ability to travel (up to 50%)

  • Meets agency health and screening requirements
