At this facility, our caregivers help prepare patients and their families physically, emotionally and spiritually prepare for the end of life. Here, you will work alongside compassionate physicians, nurses and other healthcare staff to offer patients support and comfort as they adjust to the hospice environment. 

Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not a cure, and quality of life, not duration. As a Home Health Aide on our Hospice team, you will care for patients where they need you most, whether at home, a nursing facility, assisted living center or other location. In this role, you can help enhance their outlook on their experience, even when their futures seem uncertain. Every day, you can make a lasting difference in both the lives of your patients and their loved ones. 

A caregiver in this position works 6:00pm to 6:30am on 3-12 hour shifts. Requirements include every other holiday and weekend.  

A caregiver who excels in this role will:  

  • Perform basic patient care activities.  
  • Assist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate.  
  • Assist patients with tending to personal care, activities of daily living and transfers/transport. 
  • Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff. 
  • Assist with keeping the patient's environment clean and orderly. 
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. 
  • Escalate change in patient condition to patient’s physician as appropriate.  

Minimum qualifications for the ideal future caregiver include:  

  • High School Diploma or GED 
  • State Tested Nurse Aide (STNA) Certification with the State of Ohio Nurse’s Aide Registry 
  • One year of clinical experience as an Aide in an acute, rehab or skilled nursing setting 
  • Valid Driver’s License (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverage  
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross within orientation period 

Preferred qualifications for the ideal future caregiver include: 

  • Hospice and Palliative Care Nurse Assistant (CHPNA) Certification 
  • Home Care experience

Physical Requirements:

  • Must be ambulatory, able to climb stairs, pull, push, and facilitate movement of the patient.
  • For field staff only: Able to drive to patients’ homes and/or facilities.
  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
  • Physical Demand requirements are in excess of those for Light Work.

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.
