About the Role

As a Part-Time Home Health SLP, you’ll deliver individualized therapy in patients’ homes, focusing on measurable outcomes and meaningful improvements in daily living. You’ll maintain a predictable two-day schedule, giving you the freedom to balance work with other priorities while making a lasting impact.

Key Responsibilities

  • Evaluate speech, language, cognitive-communication, voice, and swallowing abilities in-home

  • Develop and implement patient-centered treatment plans aligned with therapy goals

  • Deliver hands-on therapy to improve communication, cognition, and functional abilities

  • Monitor patient progress and adapt treatment strategies as needed

  • Collaborate with physicians, nurses, and interdisciplinary team members to ensure coordinated care

  • Educate patients and caregivers on home programs to reinforce therapy goals

  • Document clinical findings, treatments, and progress accurately and promptly

  • Support continuity of care and help prevent avoidable hospitalizations

What You Bring

  • Master’s degree in Speech-Language Pathology from an accredited program

  • Active state SLP license (or Educational Limited License with supervision)

  • Minimum one year of clinical experience; home health or hospice experience preferred

  • Strong clinical judgment and ability to work independently

  • Compassionate, patient-centered approach

  • Reliable transportation and valid driver’s license

  • Ability to travel within the assigned territory
