Conduct thorough evaluations of speech, language, voice, cognition, and swallowing abilities
Develop and implement personalized treatment plans in collaboration with patients and the care team
Deliver home-based therapy sessions following physician orders and evidence-based practices
Educate and empower patients and caregivers with home programs to reinforce therapy goals
Maintain accurate and timely clinical documentation
Communicate effectively with physicians, nurses, and therapists to ensure continuity of care
Identify patient changes early to prevent complications and unnecessary hospitalizations
Promote patient safety, independence, and overall quality of life
Master’s degree in Speech-Language Pathology from an accredited program
Active state SLP license (or Educational Limited License with supervision)
Minimum of 1 year of SLP experience; home health or hospice experience preferred
Strong clinical judgment, organizational skills, and attention to detail
Compassionate, patient-centered approach to care
Reliable transportation and valid driver’s license
Comfort working independently in patient homes
