Your Role

  • Conduct thorough evaluations of speech, language, voice, cognition, and swallowing abilities

  • Develop and implement personalized treatment plans in collaboration with patients and the care team

  • Deliver home-based therapy sessions following physician orders and evidence-based practices

  • Educate and empower patients and caregivers with home programs to reinforce therapy goals

  • Maintain accurate and timely clinical documentation

  • Communicate effectively with physicians, nurses, and therapists to ensure continuity of care

  • Identify patient changes early to prevent complications and unnecessary hospitalizations

  • Promote patient safety, independence, and overall quality of life

What We’re Looking For

  • Master’s degree in Speech-Language Pathology from an accredited program

  • Active state SLP license (or Educational Limited License with supervision)

  • Minimum of 1 year of SLP experience; home health or hospice experience preferred

  • Strong clinical judgment, organizational skills, and attention to detail

  • Compassionate, patient-centered approach to care

  • Reliable transportation and valid driver’s license

  • Comfort working independently in patient homes
