Home Health Scheduler – Valparaiso, IN

What you’ll do:
✔ Schedule patient visits and manage clinician calendars
✔ Coordinate Start of Care, follow-ups, and recertifications
✔ Communicate with CTMs, clinicians, and physicians
✔ Adjust schedules in real time based on patient needs
✔ Ensure visits are completed timely and accurately
✔ Support workflow through Homecare Homebase and internal systems
✔ Balance staffing needs with patient care demands
What you need:
✔ 1+ year scheduling or healthcare coordination experience
✔ Home health scheduling experience preferred
✔ Strong communication and problem-solving skills
✔ Ability to multitask and prioritize in a fast-paced environment
✔ Proficiency with Microsoft Office and ability to learn systems quickly
✔ High school diploma or GED required
Bonus if you have:
✔ Homecare Homebase experience
✔ Bilingual Spanish/English
