Deliver high-quality physical therapy services to patients in their homes
Evaluate patient needs, develop individualized plans of care, and monitor progress
Collaborate with physicians, caregivers, and interdisciplinary team members
Adjust treatment plans to ensure optimal functional outcomes
Supervise and guide support staff to help patients achieve therapy goals
Maintain accurate, timely documentation and treatment records
Serve as a clinical resource and advocate for physical therapy within the community
Graduate of an APTA-approved Physical Therapy program
Active, unrestricted Physical Therapist license in the state of practice
Minimum 1 year of experience in a healthcare setting
Comfortable working independently in a field-based role
Ability and willingness to travel within the assigned coverage area (approximately 50%)
Reliable vehicle, valid driver’s license, and current auto insurance
Ability to lift up to 50–100 lbs and perform physical tasks associated with patient care
