Home Health Physical Therapist

Part-Time | 2 Days per Week (16h) | Flexible Schedule

At Elara Caring, our part-time Physical Therapists play a vital role in helping patients recover, rebuild strength, and remain safely at home. This two-day-per-week position is ideal for clinicians seeking flexibility, balance, or a supplemental role—without sacrificing purpose or professional fulfillment.

The Role

As a Part-Time Home Health Physical Therapist, you’ll provide individualized therapy services to patients in their homes while maintaining a flexible, predictable two-day schedule. You’ll focus on quality care, meaningful patient relationships, and measurable outcomes—without the pressure of a full-time caseload.

What You’ll Do

  • Complete in-home physical therapy evaluations and functional assessments

  • Develop and carry out personalized treatment plans based on patient needs and goals

  • Provide hands-on therapy to improve strength, mobility, balance, and safety

  • Monitor patient progress and adjust care plans as appropriate

  • Collaborate with physicians, nurses, and interdisciplinary care team members

  • Educate patients and caregivers on exercises, safe movement, and fall prevention

  • Maintain timely, accurate clinical documentation

  • Support continuity of care and positive patient outcomes

What You Bring

  • Graduate of an APTA-accredited Physical Therapy program

  • Active, unrestricted Physical Therapist license in the state of practice

  • Minimum of one year of clinical experience (home health experience preferred)

  • Strong clinical judgment and comfort working independently

  • Reliable transportation, valid driver’s license, and current auto insurance

  • Ability to travel within the assigned territory

  • Physical ability to assist, lift, and transfer patients (up to 50–100 lbs)
