At Elara Caring, our part-time Physical Therapists play a vital role in helping patients recover, rebuild strength, and remain safely at home. This two-day-per-week position is ideal for clinicians seeking flexibility, balance, or a supplemental role—without sacrificing purpose or professional fulfillment.
As a Part-Time Home Health Physical Therapist, you’ll provide individualized therapy services to patients in their homes while maintaining a flexible, predictable two-day schedule. You’ll focus on quality care, meaningful patient relationships, and measurable outcomes—without the pressure of a full-time caseload.
Complete in-home physical therapy evaluations and functional assessments
Develop and carry out personalized treatment plans based on patient needs and goals
Provide hands-on therapy to improve strength, mobility, balance, and safety
Monitor patient progress and adjust care plans as appropriate
Collaborate with physicians, nurses, and interdisciplinary care team members
Educate patients and caregivers on exercises, safe movement, and fall prevention
Maintain timely, accurate clinical documentation
Support continuity of care and positive patient outcomes
Consistent part-time schedule with two workdays per week
Flexible scheduling within assigned service areas
One-on-one patient care in a home-based setting
Supportive leadership and clinical resources

Ideal for clinicians balancing multiple roles or transitioning schedules
Graduate of an APTA-accredited Physical Therapy program
Active, unrestricted Physical Therapist license in the state of practice
Minimum of one year of clinical experience (home health experience preferred)
Strong clinical judgment and comfort working independently
Reliable transportation, valid driver’s license, and current auto insurance
Ability to travel within the assigned territory
Physical ability to assist, lift, and transfer patients (up to 50–100 lbs)
