At Elara Caring, we’re redefining what a full-time therapy career can look like. This Physical Therapist role offers a flexible four-day workweek, giving you the ability to design a schedule that supports your life while continuing to deliver meaningful, one-on-one care to patients in their homes.
As a Home Health Physical Therapist, you will provide individualized, in-home therapy services that help patients regain mobility, independence, and confidence. You’ll manage a full-time caseload across four workdays, allowing for improved work-life balance without compromising quality or outcomes.
Conduct in-home physical therapy evaluations and functional assessments
Develop and implement personalized treatment plans aligned with patient goals and physician orders
Provide hands-on therapy focused on strength, balance, gait, and mobility
Modify care plans based on patient progress and changing clinical needs
Collaborate with physicians, nurses, and interdisciplinary team members
Educate patients and caregivers on safe mobility, exercises, and fall prevention
Complete timely, accurate clinical documentation in accordance with agency standards
Serve as a clinical resource within the communities you support
Full-time position with a flexible four-day workweek (days just need to be consistent)

Graduate of an APTA-accredited Physical Therapy program
Active, unrestricted Physical Therapist license in the state of practice
Minimum of one year of clinical experience (home health experience preferred)
Strong clinical judgment and comfort working independently
Reliable transportation, valid driver’s license, and current auto insurance
Ability to travel within the assigned territory
Physical ability to lift, transfer, and assist patients (up to 50–100 lbs)
