Key Responsibilities:

  • Provide nursing care to patients in their homes, including administering medications, wound care, and other treatments.

  • Monitor patients’ conditions, report changes to the care team, and assist in developing personalized care plans.

Well Care’s RNs work under the supervision of the Clinical Manager to provide individualized patient care according to the established policies, procedures, and guidelines.

This position is responsible for creating the individualized plan of care for patients in all stages of life in all developmental stages throughout the life span including: Neonates - 0-28 days, Infant - 1-12 months, Child - 1-12 years, Adolescent - 13-17 years, Adult - 18-72 years, Geriatric - 72 + years, according to established policies, procedures, and guidelines. Performs patient evaluations and treatments according to procedural guidelines for home health patients referred to Occupational Therapy. Participates in professional responsibilities such as program development, student programs, and performance improvement programs. Provides patient consultation and communicates with other professionals. This position is responsible for the occupational therapy of patients in all stages of life in their home. Performs other appropriate duties as assigned

S/he will be required to provide care to 5-6 patients daily in patients homes and complete documentation on the company issued laptop by the end of each day.

Job Requirements:
• Graduate of an accredited or approved school of nursing, either an AD, Diploma, or BSN program.
• Current license to practice professional nursing in the State of North Carolina.
• Significant standing and walking. Frequent lifting greater than 50 pounds and transferring patients greater than 100 pounds.
• Repeated reaching in front of the body, bending and stopping.
• Must have current NC Drivers License with clean driving record.
• Must have an operational vehicle.
