Location:           LIFE St Francis Healthcare, Wilmington, DE

Status:               Full time

Shift:                  Monday – Friday 7 am to 3:30 pm and every other weekend

Incentives:        1st Day Health Benefits, $2500 Sign On Bonus

             

Position Purpose:

The Home Health Aide is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants.

Position Details:

This position qualifies for a $2500 sign on bonus with a 2-year commitment.  The 3 installments are paid out over the two-year commitment period. 

This position will work primarily in private homes providing home care. However, some assignments in the clinic will also be required.

Holiday rotational coverage will also be required.

What you will do:

  • Provide personal care assistance that will maintain the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming.

  • Assist in the maintenance of physical activity, exercise, and walking.

  • Participant assistance may be required in the center, clinic, or home health/In Home Services department.

  • Report on personal care services provided and the self-care abilities of participant, including changes noted in participant’s condition.

  • Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance.

  • Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

  • Maintain current written records, indicating ongoing documentation of services provided, reassessments of changing needs, and participants expressed wishes.

  • Escort participants to medical, dental, optometry, rehabilitation, and other appointments as necessary.

Minimum Qualifications:

  • High school diploma or equivalent required.

  • Active Home Health Aide or Certified Nurse Assistant Certification in the state facility is in preferred.

  • Active CPR Certification required.

  • Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.

  • Experience in a long-term care facility or with a community-based geriatric program preferred.

  • Valid driver's license, proof of car insurance and ability to provide a reliable means of transportation required for home care duties.

  • Must manage conflict well, be customer focused, flexibility to adapt to ongoing change ability to work with minimal supervision and exercise independent judgment.

  • Demonstrates superior written and verbal communication and presentation skills appropriate for audience and ability to communicate effectively with individuals and groups representing

  • diverse perspectives.

  • Comprehensive to expert proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system.

  • Position may require frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting.  Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
